IF YOUR PREFERRED PHARMACY CANNOT MEET YOUR NEEDS, PLEASE CONTACT US WITH THE NAME OF PHARMACY THAT CAN FILL YOUR PRESCRIPTION NEEDS SO THAT WE MAY REDIRECT YOUR PRESCRIPTION ACCORDINGLY.
*PRESCRIPTION NEEDS WILL NEED TO BE ADDRESSED DURING OFFICE HOURS – 8:00 a.m. to 4:45 p.m.
Please be sure to register for the patient portal to access or request patient information, as well as communicate with staff.
