We are pleased to announce the addition of our new Care Manager – Kathleen Martin, RN, BS. Kathleen joins the practice with years of experience in patient care and education, supporting & working with patients and families, to achieve success in managing patient health & wellness. Kathleen completed her Associates Degree in Nursing from Mid-Michigan Community College in 2002, graduating in 1997 from Central Michigan University with Bachelor of Science degree.
Please Note, If submitting forms to be filled out (i.e. Health Appraisals, Sports physicals, etc.) we allow up to 2 weeks to be processed.
DUE TO SUPPLY CHAIN SHORTAGES OF TAMIFLU, SELECT ANTIBIOTICS, ADHD MEDICATIONS, ETC. PLEASE CONTACT YOUR PREFERRED PHARMACY TO VERIFY YOUR PRESCRIPTION CAN BE FILLED.
IF YOUR PREFERRED PHARMACY CANNOT MEET YOUR NEEDS, PLEASE CONTACT US WITH THE NAME OF PHARMACY THAT CAN FILL YOUR PRESCRIPTION NEEDS SO THAT WE MAY REDIRECT YOUR PRESCRIPTION ACCORDINGLY.
*PRESCRIPTION NEEDS WILL NEED TO BE ADDRESSED DURING OFFICE HOURS – 8:00 a.m. to 4:45 p.m.
